Job Title: Registered Nurse – Homecare/Hospice

Schedule: Monday - Friday, 8:00 AM - 4:00 PM, with one weekend per month and potential floating across counties (Morris, Somerset, Union, and Essex).

Pay Range: $63.00 - $67.00 an hour

Job Summary:

We are looking for a compassionate and skilled Registered Nurse (RN) specializing in Homecare and Hospice to join our team. The RN will perform in-depth patient assessments, develop individualized care plans, and manage ongoing patient care. Key responsibilities include case management, interdisciplinary communication, and a focus on age-appropriate, evidence-based care.

Key Responsibilities:

Patient Assessment & Care Planning:

  • Conduct thorough patient assessments to evaluate home care appropriateness, learning needs, and progress.
  • Develop, implement, and update patient-specific plans of care based on goals and evidence-based practices.
  • Accurately complete OASIS assessments and ensure daily data transfers within the required timeframe.
  • Complete discharge and re-visit documentation promptly and maintain productivity standards.

Documentation & Compliance:

  • Maintain accurate and timely documentation, ensuring alignment with insurance and regulatory requirements.
  • Demonstrate understanding of insurance reimbursement and home care regulations to optimize service use.
  • Transfer laptop data twice daily for efficient record-keeping.

Case Management:

  • Oversee case management, coordinating interdisciplinary care with team members, including the Interdisciplinary Team (IDT).
  • Supervise Home Health Aides (HHA) per standards and follow scheduling processes.
  • Prioritize care needs based on patients’ requirements, ensuring effective communication of progress with IDT.

Patient & Family Education:

  • Educate patients and families using age-appropriate methods and document all patient education tools utilized.
  • Evaluate and adjust teaching plans as necessary to meet changing patient needs.

Medication Management & Safety:

  • Reconcile medications regularly, maintaining current medication orders and demonstrating knowledge of safe pharmacology practices.
  • Ensure patient and co-worker safety by following infection control measures and maintaining a safe care environment.

Team Meetings & Activities:

  • Participate in IDT meetings, team discussions, and Performance Improvement (PI) activities as required.
  • Be flexible and willing to take on additional assignments as needed.

Qualifications:

Education:

  • Graduate of an accredited nursing program; BSN or progress towards BSN is preferred.

Experience:

  • Minimum of 2 years of experience in home care with a Medicare-certified agency; Hospice experience is a plus.

Skills:

  • Proficiency with Homecare Homebase and other relevant software.
  • Knowledge of age-appropriate care interventions and the ability to work with diverse populations.

Additional Information:

This role requires occasional weekend work (one weekend per month) and may involve travel across Morris, Somerset, Union, and Essex counties, with the greatest need in Morris County.
