Mosaic is looking for a  Registered Nurse (RN) or Licensed Practical Nurse (LPN)  to join our team!

In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records.

What YOU’LL will do:

• Coordinate physician and health care services for people served
• Conduct health care assessments and write health care plans
• Facility training and orientation of new staff regarding healthcare procedures
• Provide on call assistance to staff as needed

SCHEDULE: Days, 40 hours/week, NO WEEKENDS or HOLIDAYS

WAGE RANGE: $29-$33 RN / LPN $22-$24

What YOU’LL Need:

• BSN or ASN degree in nursing. 
• Active RN licensure for the state without restrictions.
• 1 year of general nursing experience is required OR If new LPN 3 years previous CNA or Med Aide experience
• Certified in CPR/First Aid
• Valid driver's license
• Ability to lift, push/pull and transfer 50 pounds.
