Description Mountain View Hospital is looking for a Non Clinical Care Coordinator to join our team!
JOB SUMMARY: Non-clinical care coordinators within the Mountain View organization will work closely with the population health team and clinical care coordinators to implement and roll out new processes within their clinic for things such as closing care gaps, improving workflows, and improving patient satisfaction. Non-clinical care coordinators will help implement and maintain successful quality initiative efforts in their clinic. This will include identification of qualifying patients, regular outreach to patients, tracking quality measures and progress, implementation of processes to improve patient outcomes while reaching contractual benchmarks related to value-based models of care.

DUTIES AND RESPONSIBILITIES: - Solid understanding of the healthcare continuum.
- Demonstrate sound judgment, patience and maintain a professional demeanor at all time.

- Computer skills: Word, Excel, Outlook, Electronic medical records software

- Stay current on CCM changes and requirements from CMS
- Other tasks as assigned by the supervisor or physicians
- Manage and track care gap reports
- Regular outreach to patients via phone, email, portal related to preventive needs
- Act as a liaison between the provider and the patient when coordinating care.
- Follow-up with and help complete identified referrals. Coordinate with referral coordinators and specialists and help to integrate recommendations from our provider. Identify barriers for lack of follow through or potential barriers to community resources.
- Scheduling of patients as needed for preventive needs/services
- Collaborate closely with population health team to close care gaps, implement new process and procedures within clinic to promote quality patient care, positive patient outcomes, and decreased provider burden.
- Follow HIPAA guidelines when coordinating care
- Maintains patient confidentiality at all times. Reports to work on time and as scheduled, completes work within designated time.
- Helps to ensure all deadlines are met during transition stages.
- Offers education and tutoring to nurses involved in the program. May also provide education to all providers if needed.
- Will participate in webinars, conferences, education meetings, and transitional calls.
- Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as a liaison between administration, patients, physicians and other healthcare providers.
- Consults other departments as appropriate for an interdisciplinary approach to the patient's needs.
- Communicates appropriately and clearly to manager, co-workers and physicians.
- Any additional floor duties deemed necessary by the nurse supervisor and manager. This would include additional training for duties such as phlebotomy, basic vitals, and patient intake.

Qualifications Education/Certification: High school diploma or equivalent.
Experience: To function effectively in this role, it is preferred that the candidate have experience in outpatient care settings. Past experience with population health is preferred.
Equipment/Technology: Must be able to use Microsoft Office, Word, and Excel. Adapt to computer systems quickly. Comfortable learning and adapting to new software systems.
Language/Communication: Must have the sensitivity, maturity, and ability to communicate clearly and concisely, both verbally and in writing. Must be patient, courteous, compassionate, and respectful with patients and their families, co workers, physicians and administrative staff. Ability to communicate and relate well with physicians, staff and management.
Mathematical Skills: General mathematics
Mental Capabilities: Ability to apply common sense to carry out instruction furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. A clear understating of the importance of patient confidentiality and sensitivity regarding complex health problems. Maintain a positive attitude and willingness to jump in and help as you identify needs and upon request of the providers and team members.
Interpersonal: Ability to work well with others as a team player. Self-motivated, self-starter and initiative in establishing and developing the role of a non-clinical care coordinator. Organized, detail oriented, and the ability to manage a heavy load of various tasks. Time management skills are a must. Maintain a neat and professional appearance and workspace.
