Salary Range: $98,556.64- $128,775.92 RN with greater than 5 years of ICU experience starting salary: $128,775.92 Job Type: Full-time nights Hours: 12-hour shifts; 7 days in a two week period, including alternating weekends. Duties and Responsibilities: Referral Management Process: Responds promptly and appropriately to potential donor referrals to assess suitability and enhance the donation process. Establishes and enhances relationships with the hospital staff to increase referral activity. Introduces oneself to the referred patient’s bedside Nurse and the units Charge Nurse to establish presence. When leaving, advise the bedside Nurse and Charge Nurse of impending departure and inquire if there is anything needed before leaving the facility. Collaborates with the RTS (Referral Triage Specialist), CRS (Clinical Resource Specialist), SOP (on call Supervisor of Organ Procurement), Medical Director of Referral Management and/or clinical leadership to manage active referrals. Accesses and reviews the referring hospital’s medical records charts. Transcribe appropriate information to digitalDONOR fields per OneLegacy policies and practices. Consults with hospital staff to evaluate current organ function and identify early stages of medical derangements and current organ function. Formulates a treatment plan in the direction of the Medical Director of Referral Management. Advocates on behalf of potential donors and their families for interventions needed to preserve the opportunity for donation and optimize organ function. Makes written and oral reports/recommendations to the attending physician, transplant physician, Medical Director, consulting Intensivist and CRS concerning the status and care of the donor, as appropriate. Develops and communicates a plan for continuity of care along with the progression of the referral. Reviews medical-legal documentation pertaining to brain death declaration for completeness and accuracy according to hospital policy, California Health and Safety codes and current American Association of Neurology Guidelines. When revisions are needed, provides direction to appropriately complete documentation. Sends acceptable brain death documentation to the RTS and CRS to be verified per OneLegacy policy. Performs administrative duties regarding the patient, such as completing digitalDONOR charting, as well as any other forms or documentation required per OneLegacy documentation policies and/or hospital policies. Implements and maintains established policies, procedures, objectives, quality assurance, safety, environmental and infection control; reports these to the transplant center, physician, resident, family members and public, as appropriate. Family Support Process: Identifies AP (Authorizing Party) and language requirements. Documents pertinent family dynamics and any other relevant information needed to properly assess the AP and/or family members of potential donors in preparation for approach. Collaborates with the hospital care team, the RTS and/or the Family Care Specialist Resources (FCSR) in the end-of-life discussion to ensure the AP is provided with the opportunity for donation in the most appropriate manner. Discusses the elements of authorization with the AP and/or family members of potential donors. Obtains authorization for donation per UAGA and verifies medical-legal documentation necessary for organ recovery is complete according to OneLegacy policies and procedures. Completes Donor Risk Assessment Interview (DRAI) and obtains any required follow-up information. Organ Donation Process: Responds to donor referrals or assigned donor cases at shift start time unless otherwise instructed CRS/RTS. Obtains accurate medical information for evaluation and evaluates potential organ and tissue donors according to OneLegacy, regulatory and/or credentialing body guidelines. Discusses and supports the donation process with family members or Authorizing/Acknowledging Party (AP). Coordinates all aspects of donor management. Assures accuracy and completeness of charting in the hospital and OneLegacy Electronic Medical Record systems. Collaborates with Medical Examiner/Coroners, tissue recovery staff, hospital staff, social services and clergy. Coordinates the logistics for multiple organ procurements and allocation cases. Maintains consistent and clear communication with Donor Allocation Coordinator (DAC) regarding organ allocation. Verifies that all medical-legal documentation necessary for organ procurement is completed according to OneLegacy, governing and/or credentialing body guidelines. Provides organ allocation staff with organ/donor information, as required. Responsible for all chart documentation in the OR during recovery and post recovery including review and completion of Confidential Donor File (CDF) at the conclusion of each completed case. Donation Development Process: Reviews Not Brain Dead Status Board (NBD) when in a hospital for an active referral. Conduct a site visit on NBD referrals and chart in EMR system. Participates in “Presence with a Purpose” in collaboration with the Donation Development Coordinator (DDC) to educate hospitals on the donation process along with current and upcoming donation related events. Coordinates and conduct scheduled education on donation and transplantation in partnership with the DDC in assigned hospital(s). Participates in OneLegacy signature events such as: OneLegacy Donation and Transplantation Symposium, Donate Life RunWalk, etc. as requested. Promotes positive relationships with all hospital and clinical staff. Participates in hospital meetings to assist with presenting information or addressing issues relating to the donation process. Joins hospital sponsored clinical round and/or conducts other case-finding activities. Debriefs Donation Development Coordinator (DDC) after a case, as needed. Provides clinical consultation for meeting preparation and reports. OneLegacy Recovery Center Responsibilities: The PTC-II will maintain proficiency and competency in all organ operations at the OneLegacy’s Recovery Center, Appendix A. Personnel Functions: Always maintain professional conduct with both internal and external customers. Cooperates with other personnel to achieve company objectives and maintains good employee, interdepartmental and public relations. Meets with and solicits advice from the Medical Director(s), Director of Organ Clinical Operations, Supervisor, Donor Management, SOP (Supervisor on call), CRS, Transplant Centers, Attending Physicians, Director of Nursing Services and Nursing Staff concerning donor patient care. Maintains an effective and friendly working relationship with health professionals, physicians, consultants, and family members involved in every donation process. Work Schedule Responsibility: Shares schedule responsibility with other assigned PTC’s (seven 12-hour shifts per pay period every two weeks). Additional work time and shifts are assigned, as needed, to uphold OneLegacy’s mission. Will be required to travel within OneLegacy’s Donor Service Area to respond to referral/donor cases or other OneLegacy business. Gives verbal report to oncoming OneLegacy staff when assignment is completed. Carries OneLegacy issued electronic devices and appropriate case materials on scheduled work shifts and ensure that all equipment is working properly. Enables and ensures location services on OneLegacy issued devices work properly during scheduled shifts. Other Responsibilities: Stays professionally current as required for assigned job function, certification, or licensure. Complies with all UNOS policies, standards, and requirements. Attends staff meetings and scheduled trainings unless on call, post-call, or protected time off (UPTO/PTO, bereavement, jury duty, etc.) as deemed necessary by Leadership. Ability to perform patient transfers and heavy lifting up to 50 pounds. Ability to function as an effective and competent team member. Maintain confidentiality of all information pertinent to organ, eye and tissue donors and recipients, OneLegacy staff, OneLegacy affiliated donor hospitals and affiliated processors. Physical Environment/Working Conditions: Location: OneLegacy Donor Service Area (DSA) with assigned regions including Corporate (Los Angeles), Bakersfield, Orange, and Redlands. Travel: Required to travel by personal auto or air to meeting sites and other locations. Work Hours: Shifts are scheduled for 12 hours, with typical shifts between 08:00-20:00/20:00-08:00. Work hours can vary by service area and due to fluctuations in donor activity. Assigned day and/or shift can be altered according to staffing needs. PTC-II staff must be available evenings, holidays, and weekends as required, and includes working overtime as approved and required. Working Conditions: See ADA Worksheet. Job Qualifications: Education: Meet all qualifications for Procurement Transplant Coordinator-I (PTC-I) Experience: Demonstrated proficiency with supporting documentation in all advance procedures to be eligible for APC Certification/License: Required to be a PTC-II within 24 months of employment. Basic Cardiac Life Support (BCLS) certification required at the time of employment. Required to be Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certified within the employee’s orientation period.
