Case Manager can expect to see 20-30 patients per week. Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.\nEPIC/Computer charting. Sapphire IV pumps. T\n\nCoverage:\nEast to West: Mountgreenwood to Willow Springs \nNorth to South: Burbank to Chicago Ridge \nZip codes: 60459, 60415, 60455, 60453, 60456, 60457, 60458, 60480, 60652, 60655, 60655, 60805, 60805, 60499, 60501\n\nCandidate MUST HAVE:\n- 1 year Home Health RN experience\n- Active BLS (AHA) and state RN license (IL)\n- Wound care experience (wound vacs, pleural drains and ostomy care).\n- Able to complete peripheral blood draws, central line care, and IV admin through multiple devices (elastomeric, IV pumps).\n- Excellent assessment skills.\n- Experience with computer documentation.\n- Reliable transportation, active driver’s license, and insurance.
