SUMMARY STATEMENT:

The Community Health Worker (CHW), under the supervision of the Supervisor and President, will participate and serve as the primary change agent of the Office of Diversion and Reentry Intensive Case Management Services (RICMS). CHW will provide connections to direct services, care coordination and system navigation, coaching, and social support as well as education about the health and social service system, advocacy, outreach, assessment, and capacity building. Additionally, will serve as liaison and cultural mediators, continuously educate members of the health and social service system about community strengths, and needs, and may participate in research/evaluations.

DUTIES AND RESPONSIBILITIES:

CONDUCT OUTREACH, ADVOCACY, AND A REFERRAL NETWORK FOR PROJECT SERVICES 

• Provide health and wellness support to reentry clients by providing intensive case management services.
• Serve as an advocate on behalf of the participant within justice systems, clinical, and community-based settings to help the participant achieve their goals: (i.e. transporting the client to and from meetings and appointments).
• Connect and engage participants in community services and activities. 
• Build and maintain trusting and open relationships with community organizations, leaders, and resources.
• Conduct assessments of participant strengths and needs. This includes administering appropriate screening and/or assessment tools.
• Provide support to clients and family members on an as-needed basis.
• Assist participants in setting goals related to health and wellness, housing, legal services, employment, and self-sufficiency.
• In conjunction with other team members and each participant, assist with evaluating progress towards goals and make adjustments in the case management plan to facilitate progress towards goals.
• Complete all necessary and required documentation, which includes the use of the Case Management Platform database.
• Compile and report summary program data during weekly check-in calls, assessments, and quarterly meetings as well as staff meetings.
• Maintain participant confidentiality and privacy by protecting participant health information.
• Participate in team-building efforts to promote positive intrapersonal relationships with team members

QUALIFICATIONS:

REQUIRED

• High school diploma or equivalent.
• Lived experience, meaning have been incarcerated in the criminal justice system.
• Ability to work appropriately, effectively, professionally, and adapt to the program requirements.
• Permanent resident or U.S. citizen; proof of work eligibility upon hiring.
• Valid Class C Driver’s license, California (Must be willing to drive clients to appointments and or drop off essential goods. Minimum automobile insurance, and an appropriate motor vehicle.
• Bilingual Spanish is preferred.
